Losing strength. Not feeling like yourself. Your sex drive\u2019s off. Your doctor brings up testosterone therapy.&nbsp;<\/p>\n\n\n\n<p>Sounds tempting: more energy, better mood, sharper workouts, stronger libido.&nbsp;<\/p>\n\n\n\n<p>Then comes the catch: What does this mean for my heart?&nbsp;<\/p>\n\n\n\n<p>That\u2019s the question a lot of men are asking\u2014and it\u2019s a good one. Cardiovascular disease is still the #1 killer of men. If you\u2019re going to mess with your hormones, you want to know what it\u2019s doing to your arteries, your blood, and your long-term health.&nbsp;<\/p>\n\n\n\n<p>This isn\u2019t hype. It\u2019s about making an informed call\u2014based on the real science, not the marketing promises or the horror stories online.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-a-personal-perspective-on-trt-nbsp\">A Personal Perspective on TRT&nbsp;<\/h2>\n\n\n\n<p>Here\u2019s how I look at it: testosterone therapy can be beneficial for some men, but for most, it\u2019s a \u201cdeal with the devil\u201d. Once you\u2019re on it, you\u2019re often on for life. That\u2019s not always obvious up front, but the long-term physiological consequences, like suppressed natural production, tend to surface as therapy continues. I lay that out in detail in my book, <a href=\"https:\/\/craigcooper.net\/t\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>Your New Prime<\/em><\/a>.&nbsp;<\/p>\n\n\n\n<p>At 62, I\u2019m fortunate to have a total testosterone level of 854 ng\/dL, within a lab range of 0 to 1000, with free T significantly above the normal threshold. But that didn\u2019t come from a prescription. And I know others with clinically low testosterone who are thriving and bouncing off walls. Energy, vitality, sex drive, mood\u2014these are driven by more than just your bloodwork. The number is only one piece of the puzzle. What matters is how you\u2019re actually functioning, day in and day out.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-trt-isn-t-a-shortcut-it-s-a-medical-treatment-nbsp\">TRT Isn\u2019t a Shortcut. It\u2019s a Medical Treatment.&nbsp;<\/h2>\n\n\n\n<p>Testosterone therapy isn\u2019t about turning back the clock or building a beach body. It\u2019s meant for one thing: treating clinical hypogonadism. That\u2019s when your body isn\u2019t producing enough testosterone and you\u2019re feeling the effects.&nbsp;<\/p>\n\n\n\n<p>That means:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You\u2019ve got consistently low testosterone on bloodwork (measured in the morning, multiple times): <em><strong>and<\/strong><\/em><\/li>\n\n\n\n<li>You\u2019ve got symptoms: fatigue, low libido, mood swings, declining muscle mass, belly fat that won\u2019t budge&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>If you don\u2019t check both boxes, TRT probably isn\u2019t for you. In fact, some estimates suggest that as many as one in three men prescribed TRT don\u2019t actually meet the criteria for clinical deficiency. That\u2019s where the risk skyrockets\u2014because for men with normal T levels, the benefits are minimal, but the downsides still apply. And even if you do, it\u2019s not a silver bullet. <a href=\"https:\/\/www.mayoclinic.org\/healthy-lifestyle\/sexual-health\/in-depth\/testosterone-therapy\/art-20045728\" target=\"_blank\" rel=\"noreferrer noopener\">It\u2019s one piece of a larger health strategy.<\/a>&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-testosterone-does-for-your-cardiovascular-and-musculoskeletal-systems-nbsp\">What Testosterone Does for Your Cardiovascular and Musculoskeletal Systems&nbsp;<\/h2>\n\n\n\n<p>Testosterone doesn\u2019t just affect your sex drive. Improves Blood Vessel Function<\/strong>&nbsp;<\/p>\n\n\n\n<p>Testosterone helps blood vessels expand. That matters for blood flow and blood pressure. Low T can reduce this function; <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S2050052119300137\" target=\"_blank\" rel=\"noreferrer noopener\">TRT may help restore it<\/a>.&nbsp;<\/p>\n\n\n\n<p><strong>2. Reduces Visceral Fat<\/strong>&nbsp;<\/p>\n\n\n\n<p>The deep belly fat that surrounds your organs is tied to insulin resistance and inflammation\u2014both risk factors for heart disease. <a href=\"https:\/\/www.healio.com\/news\/endocrinology\/20230222\/testosterone-therapy-may-improve-body-composition-lipids-for-men-with-prediabetes\" target=\"_blank\" rel=\"noreferrer noopener\">TRT can help shift fat distribution and build lean mass<\/a>.&nbsp;<\/p>\n\n\n\n<p><strong>3. Supports Glucose Metabolism<\/strong>&nbsp;<\/p>\n\n\n\n<p>TRT may help regulate blood sugar. <a href=\"https:\/\/www.healio.com\/news\/endocrinology\/20230222\/testosterone-therapy-may-improve-body-composition-lipids-for-men-with-prediabetes\" target=\"_blank\" rel=\"noreferrer noopener\">One large trial (T4DM)<\/a> showed it could delay or prevent diabetes in men at risk\u2014good news for your heart.&nbsp;<\/p>\n\n\n\n<p><strong>4. Lowers Inflammation<\/strong>&nbsp;<\/p>\n\n\n\n<p>Low testosterone is linked to higher levels of inflammatory markers. TRT can bring those down, which may help <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17468196\/\" target=\"_blank\" rel=\"noreferrer noopener\">reduce arterial plaque buildup<\/a>.&nbsp;<\/p>\n\n\n\n<p><strong>5. Improves Exercise Capacity<\/strong>&nbsp;<\/p>\n\n\n\n<p>You may find you can <a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2024.06.21.24309326v1.full.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">train harder and recover faster<\/a>, which encourages a more active lifestyle\u2014key for long-term heart health.&nbsp;<\/p>\n\n\n\n<p><strong>6. Supports Bone Health<\/strong>&nbsp;<\/p>\n\n\n\n<p>TRT may also help increase bone mineral density, particularly in men with osteoporosis or age-related decline. However, in the TRAVERSE trial, a small but unexplained increase in bone fractures was observed\u2014highlighting the need for longer-term studies.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-risks-are-real-if-you-re-not-monitoring-nbsp\">The Risks Are Real\u2014If You\u2019re Not Monitoring&nbsp;<\/h2>\n\n\n\n<p>None of this means TRT is risk-free. It shifts your physiology in ways that require oversight. If you\u2019re on therapy and not watching the right markers, you\u2019re flying blind.&nbsp;<\/p>\n\n\n\n<p>Here\u2019s what can go wrong:&nbsp;<\/p>\n\n\n\n<p><strong>1. Blood Gets Too Thick<\/strong>&nbsp;<\/p>\n\n\n\n<p>TRT increases red blood cell production. If hematocrit goes above 54%, your blood can get too viscous, raising clot and stroke risk. This condition is called <strong>erythrocytosis<\/strong>, and it\u2019s&nbsp;one of the <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5796953\/\" target=\"_blank\" rel=\"noreferrer noopener\">most common issues<\/a> . It\u2019s manageable\u2014but only if you\u2019re tracking it.&nbsp;<\/p>\n\n\n\n<p><strong>2. Blood Clot Risk<\/strong>&nbsp;<\/p>\n\n\n\n<p>Most recent studies, including the 2023 TRAVERSE trial, show no major increase in clot risk overall\u2014but there was a small <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2215025\" target=\"_blank\" rel=\"noreferrer noopener\">uptick in pulmonary embolisms<\/a>. If you\u2019ve had clots before, this matters.&nbsp;<\/p>\n\n\n\n<p><strong>3. Water Retention and Blood Pressure<\/strong>&nbsp;<\/p>\n\n\n\n<p><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3440621\/\" target=\"_blank\" rel=\"noreferrer noopener\">Some men retain fluid on TRT<\/a>. This can elevate blood pressure or cause swelling\u2014especially if you already have hypertension.&nbsp;<\/p>\n\n\n\n<p><strong>4. Irregular Heart Rhythm (AFib)<\/strong>&nbsp;<\/p>\n\n\n\n<p>The same 2023 study noted a <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2215025\" target=\"_blank\" rel=\"noreferrer noopener\">slight increase in atrial fibrillation<\/a>. Rare, but important\u2014especially if you\u2019ve had rhythm issues before.&nbsp;<\/p>\n\n\n\n<p><strong>5. Worsened Sleep Apnea<\/strong>&nbsp;<\/p>\n\n\n\n<p><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10597633\/\" target=\"_blank\" rel=\"noreferrer noopener\">TRT can make sleep apnea worse<\/a>. If you snore heavily or feel exhausted even after a full night, get it checked before starting therapy.&nbsp;<\/p>\n\n\n\n<p><strong>6. Infertility and Testicular Shrinkage<\/strong>&nbsp;<\/p>\n\n\n\n<p>TRT suppresses natural testosterone production and can significantly reduce sperm count\u2014sometimes permanently. Long-term use may cause testicular shrinkage that isn\u2019t reversible.&nbsp;<strong>Sperm recovery can take up to two years after stopping, and medications like clomiphene or hCG may help, though they\u2019re not always covered by insurance.<\/strong>&nbsp;<\/p>\n\n\n\n<p><strong>7. Gynecomastia and Acne<\/strong>&nbsp;<\/p>\n\n\n\n<p>Testosterone is converted into estrogen in the body. That means some men on TRT may develop gynecomastia\u2014painful swelling or tenderness of male breast tissue. Others see a resurgence of acne, particularly on the back and shoulders.&nbsp;<\/p>\n\n\n\n<p><strong>8. Prostate Concerns<\/strong>&nbsp;<\/p>\n\n\n\n<p>While TRT hasn\u2019t been definitively linked to prostate cancer, it can enlarge the prostate and worsen symptoms of benign prostatic hyperplasia (BPH)\u2014think frequent urination, urgency, and disrupted sleep. Monitoring PSA levels and discussing prostate health with your doctor is critical.&nbsp;<\/p>\n\n\n\n<p><strong>9. Emotional and Behavioral Changes<\/strong>&nbsp;<\/p>\n\n\n\n<p>Some men experience increased irritability, impulsivity, or even depressive symptoms with testosterone fluctuations\u2014especially if therapy isn\u2019t well-regulated. Mood tracking should be part of any long-term TRT plan.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-so-is-trt-safe-for-your-heart-nbsp\">So, Is TRT Safe for Your Heart?&nbsp;<\/h2>\n\n\n\n<p>Short answer: For men with true low testosterone\u2014yes, it appears to be safe, when monitored properly.&nbsp;<\/p>\n\n\n\n<p>That <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2215025\" target=\"_blank\" rel=\"noreferrer noopener\">2023 TRAVERSE trial<\/a> followed over 5,000 men with cardiovascular risk for nearly three years. No increase in heart attacks, strokes, or cardiac deaths. That\u2019s the most rigorous data we have to date.&nbsp;<\/p>\n\n\n\n<p>But it\u2019s not a green light to self-medicate or skip the follow-ups. Testosterone affects multiple systems. If you\u2019re on it, you need structure.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-to-use-trt-without-putting-your-heart-at-risk-nbsp\">How to Use TRT Without Putting Your Heart at Risk&nbsp;<\/h2>\n\n\n\n<p><strong>1. Confirm You Actually Need It<\/strong>&nbsp;<\/p>\n\n\n\n<p>Blood tests <strong><em>plus <\/em><\/strong>symptoms. Not just one or the other.&nbsp;<\/p>\n\n\n\n<p><strong>2. Choose a Delivery Method That Works for You<\/strong>&nbsp;<\/p>\n\n\n\n<p>Injections can spike levels fast, which may raise red blood cell counts. Gels and patches tend to provide steadier levels.&nbsp;<\/p>\n\n\n\n<p><strong>3. Get the Right Labs\u2014and Get Them Regularly<\/strong>&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hematocrit &amp; Hemoglobin (3 and 6 months, then yearly)&nbsp;<\/li>\n\n\n\n<li>Testosterone levels (to stay mid-normal)&nbsp;<\/li>\n\n\n\n<li>PSA &amp; digital prostate check&nbsp;<\/li>\n\n\n\n<li>Blood pressure (home + clinic)&nbsp;<\/li>\n\n\n\n<li>Lipid panel&nbsp;<\/li>\n\n\n\n<li>Fasting glucose &amp; A1C&nbsp;<\/li>\n\n\n\n<li>Sleep and cardiac symptoms (snoring, fatigue, palpitations)&nbsp;<\/li>\n<\/ul>\n\n\n\n<p><strong>4. Bring Your Cardiologist Into the Loop<\/strong>&nbsp;<\/p>\n\n\n\n<p>If you\u2019ve got heart history, don\u2019t manage TRT in isolation. Watch your sodium.&nbsp;<\/li>\n\n\n\n<li><strong>Recovery:<\/strong> Sleep and stress matter\u2014especially if you\u2019re pushing hard in the gym.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-bottom-line-nbsp\">The Bottom Line&nbsp;<\/h2>\n\n\n\n<p>TRT isn\u2019t a magic bullet. But for the right guy\u2014diagnosed properly, monitored consistently\u2014it can make a real difference.&nbsp;<\/p>\n\n\n\n<p>Energy improves. Fat goes down. Muscle comes back. Libido often improves. And with the right structure, your heart doesn\u2019t have to suffer for those gains.&nbsp;<\/p>\n\n\n\n<p>But it only works if you respect the therapy.&nbsp;<\/p>\n\n\n\n<p>No shortcuts. No guessing. And definitely no ignoring your labs or skipping follow-ups.&nbsp;<\/p>\n\n\n\n<p>Ask yourself:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Am I a real candidate for TRT?&nbsp;<\/li>\n\n\n\n<li>Am I prepared to track my heart health and adjust as needed?&nbsp;<\/li>\n\n\n\n<li>Am I handling the fundamentals\u2014exercise, food, sleep, stress?&nbsp;<\/li>\n\n\n\n<li>Am I working with the right team?&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>Because TRT isn\u2019t just about hormones.
