I decided to treat it as feedback and changed what I could control. Here’s what worked, what I added later, and where the number landed.

In a Heartbeat

The result: From December to July, my ApoB dropped from 110 to 79 mg/dL, a 28% reduction, and my LDL-C fell 44%.

What moved it: Lifestyle first: less saturated fat, different protein sources, 40+ grams of fiber a day, and fueling my training properly. Then my cardiologist added a medication (ezetimibe).

The honest part: Lifestyle was already working before the medication. Adding it accelerated the result, though I can’t say exactly how much came from each.

The takeaway: It’s not lifestyle versus medication. Track the number, work the fundamentals first, add a tool when it helps, and keep measuring.

Back in February, I wrote about something in my bloodwork that surprised me: my ApoB level, one of the most important blood biomarkers for assessing cardiovascular risk.

It was high, even though I’m fit. So I set out to answer a question a lot of people are asking: how to lower ApoB when you’re already doing most things right.

Since publishing that article, I’ve had quite a few emails and messages from people asking essentially the same questions:

What happened? Where is your ApoB now? Did the changes you made actually work? And what did you do?

So this is the follow-up. And the result surprised me even further.

When I first wrote about this, I was 62, lean (10-11% body fat), metabolically pretty healthy and training most days of the week. My triglycerides were 48 mg/dL, HDL was 86 mg/dL, inflammation was low, insulin sensitivity was good and my Lp(a) was low. Good by most standards.

But my LDL-C was 154 mg/dL and, more importantly, my ApoB was 110 mg/dL.

That was the disconnect.

By most conventional measures, I looked healthy. But ApoB was telling me something different: I had more atherogenic lipoprotein particles circulating in my blood than I wanted.

I could have rationalized the number because I was fit.

Instead, I decided to treat it as feedback.

What I set out to do to lower my ApoB

In that original article, I laid out what I intended to do about it. Rather than immediately assuming I needed a pharmaceutical intervention, I wanted to first see how far I could move the numbers by really leaning into the things I could control: diet, supplementation, fueling, recovery and lifestyle.

Importantly, I wasn’t trying to conduct a perfectly controlled experiment. I wanted to make a series of evidence-based changes simultaneously, stay consistent with them, and then retest.

My initial goal was a 10–25% reduction in ApoB, while maintaining my muscle, training capacity, endurance and performance.

I’m now 63.

And we have the answer.

December 15, 2025 — ApoB: 110 mg/dL

March 25, 2026 — ApoB: 101 mg/dL

July 20, 2026 — ApoB: 79 mg/dL

That’s a reduction of 31 mg/dL, or 28.2%, in approximately seven (max-width:1024px) 100vw, 1024px" />

But there’s an important part of the story behind those three data points.

Because this actually happened in two distinct phases.

The first was lifestyle.

The second was lifestyle plus medication.

And that distinction is important.

Phase One: Lifestyle was working

Between my first blood test in December and my second test in March, I had been 100% focused on lifestyle, diet and supplementation.

I hadn’t started a prescription lipid-lowering medication.

By March, my ApoB had fallen from 110 to 101 mg/dL, an 8.2% reduction.

My LDL-C was also moving in the right direction.

That was important to me.

It demonstrated that the changes I’d made weren’t just theoretically good ideas.

They were working.

Both ApoB and LDL-C were trending positively before medication entered the picture.

And that gave me a reason to keep doing what I was doing.

Phase Two: Then my cardiologist suggested we supercharge it

When I reviewed the results with my cardiologist, her advice was essentially:

“You’re doing all the right things, and it’s definitely helping. Why don’t we try adding some medication and see if we can supercharge the process?“

She suggested Zetia (ezetimibe), which she viewed as a relatively straightforward addition with a generally favorable side-effect profile.

This wasn’t medication being prescribed because lifestyle had failed.

It was medication being added because lifestyle was already working.

The question became: What happens if I keep doing everything that’s working and add another evidence-based tool on top of it?

So sometime after that March result, I started Zetia while continuing all of the dietary, supplement and lifestyle changes I had already made.

And that’s when the numbers really accelerated.

By July, my ApoB had fallen again:

101 → 79 mg/dL.

Across the full period:

110 → 101 → 79 mg/dL, a 28.2% reduction.

And my LDL-C ultimately fell by 44.2%.

I’ve stayed on Zetia since then.

So for everyone who has emailed asking what happened, the most accurate short answer is:

Lifestyle moved the numbers in the right direction, and adding medication on top of that foundation supercharged the result.

That distinction matters.

It would be misleading for me to show a 28.2% reduction in ApoB and a 44.2% reduction in LDL-C and imply that I achieved all of it through food, supplements and exercise alone.

I didn’t.

But it would be equally wrong to conclude that lifestyle didn’t matter.

Both my ApoB and LDL-C were already improving before I started Zetia.

So what exactly did I change?

1. I dramatically reduced saturated fat

This was probably the biggest dietary change I made.

I’d spent the previous year deliberately increasing my protein intake to preserve muscle and support a high training load.

But that had also meant eating more red meat (mainly bison), more full-fat dairy and typically three or four whole eggs most mornings. And because of my training load, I felt like I had a lot of moral license to eat a few treats now and then.

Individually, I don’t consider any of those foods inherently “bad.”

The issue was the total dietary pattern and how my own physiology was responding to it.

So I changed the fat composition of my diet. Here’s what that looked like in practice:

substantially reduced red meat

limited butter

substantially reduced full-fat dairy

replaced dairy in coffee, matcha and smoothies with organic soy milk

stopped eating my daily egg yolks

made extra virgin olive oil my principal added fat

increased avocado, nuts and seeds

continued eating oily fish, particularly sardines and salmon

The objective wasn’t to eat a low-fat diet.

It was to eat a much lower-saturated-fat, higher-unsaturated-fat diet.

That distinction is important. I wasn’t trying to eliminate dietary fat. I was deliberately changing the type of fat I was consuming.

2. I kept protein high, but changed where it came from

This was important because I wasn’t prepared to lower ApoB at the expense of muscle or performance.

I didn’t dramatically reduce protein.

I changed the source of that protein.

I shifted much more toward:

salmon

sardines

shrimp

soy

lean poultry

other lean protein sources

At the same time, I reduced my reliance on red meat, higher-fat dairy and egg yolks.

So the principle became:

Same, or more, protein. Very different lipid consequences.

For someone my age who trains as much as I do, maintaining muscle is an important part of my longevity strategy. I wasn’t interested in improving one biomarker by inadvertently compromising something else that matters.

3. I made soluble fiber a deliberate intervention

I’d always eaten what I considered a healthy diet, so previously fiber was something I assumed I was getting enough of.

I stopped assuming.

I deliberately increased (with no commission links to the products I use):

psyllium husk

oats

chia seeds

ground flax

vegetables

inulin and other prebiotic fiber

I targeted approximately 40+ grams of total fiber per day, with particular attention to getting enough soluble and viscous fiber.

Fiber went from being something that happened to be in my diet to something I actively used as an intervention.

That’s an important distinction because soluble and viscous fiber can help lower LDL cholesterol by reducing intestinal cholesterol absorption and increasing bile acid excretion.

It was one of the simplest changes I made, but also one of the ones I was most consistent with.

4. I added a targeted supplement strategy

I also experimented with a few supplements that have varying degrees of evidence for influencing lipid metabolism and cardiovascular health. At the same time, I simplified everything else, stopping all of my other supplements and keeping only protein and creatine.

Berberine

Approximately 500 mg twice daily.

I used this primarily as a metabolic and lipid intervention, including its potential effects on LDL-receptor activity and hepatic lipid metabolism.

Omega-3s

I continued approximately 2–3 grams of EPA+DHA daily, with an emphasis on EPA.

Omega-3s aren’t primarily an ApoB-lowering intervention. Their strongest lipid effect is generally on triglycerides, but they remained part of my broader cardiovascular strategy.

Inulin

Approximately 5–10 grams daily, primarily as additional prebiotic fiber and as another way of increasing my overall fiber intake.

Nattokinase

I also included nattokinase as part of my broader cardiovascular supplement stack.

I don’t attribute the ApoB reduction to nattokinase. It isn’t a primary ApoB-lowering intervention, and I think it’s important not to imply that it is. I view it separately, principally in the context of its fibrinolytic activity and broader cardiovascular biology.

I also continued other cardiovascular staples such as CoQ10 and magnesium.

One supplement from the original plan that didn’t make the cut: citrus bergamot. I’d floated it in February as a liver-signalling experiment.

The important point here is that supplements were adjuncts. They weren’t the strategy by themselves.

I wouldn’t look at my final result and conclude that taking a handful of supplements will lower someone’s ApoB by 28%.

That’s not what happened.

The supplements sat on top of much more fundamental changes to my diet, fat intake, fiber, protein sources, fueling and recovery.

5. I became smarter about fueling and recovery

This was one of the less obvious changes, and perhaps one of the more interesting ones for me personally.

When someone who exercises sees an unfavorable cardiovascular biomarker, the instinct can be to think:

Maybe I need to exercise more.

That clearly wasn’t my problem.

I was already training most days of the week, combining significant cardio work with strength training.

So rather than adding more exercise, I became more conscious of energy availability, fueling and, most importantly, recovery. To do that, I:

fueled harder endurance sessions more deliberately

prioritized carbohydrate and protein immediately after training

made sure my caloric intake reflected my training load

stopped treating prolonged low-energy availability as inherently beneficial

moved away from unnecessary fasting around significant training

focused more on recovery rather than simply adding additional training volume

That represented a fairly significant change in my thinking.

I’ve always been attracted to interventions that potentially improve health and longevity. But context matters.

For someone doing a significant amount of endurance training, more training and less food aren’t automatically healthier, which is a cycle I’ve been stuck in for many years.

Sometimes the healthier intervention is actually to fuel the work you’re asking your body to perform. Sounds logical, but it’s been a real issue for me in the past. I’m going to write about this in the near future with further context.

6. I didn’t chase weight loss

This wasn’t a weight-loss experiment.

I was already lean and metabolically healthy.

My triglycerides, insulin, glucose control and inflammatory markers weren’t suggesting that obesity or significant metabolic dysfunction was driving my ApoB.

So I didn’t reduce calories.

Instead, I changed the composition of what I was eating while continuing to adequately fuel my training.

My objective was very specific:

Improve the biomarker without sacrificing muscle, recovery or performance.

That is an important part of the story because “heart healthy” shouldn’t automatically mean eating less.

For me, it meant eating differently.

7. I kept doing all of this after starting Zetia

This is also important.

Adding medication wasn’t a reason to stop doing the work.

I didn’t start Zetia and abandon the diet.

I didn’t stop the fiber.

I didn’t go back to eating the way I had previously.

I didn’t stop paying attention to protein quality, fueling or recovery.

I added Zetia to a lifestyle strategy that was already producing a measurable result.

That sequence is important to me:

Lifestyle first: ApoB 110 → 101.

Lifestyle + Zetia: ApoB 101 → 79.

8. Most importantly, I measured instead of guessing

This is probably the biggest lesson from the entire experiment.

I started with an objective measurement:

December 15, 2025 — ApoB: 110 mg/dL

Then came approximately three months focused entirely on lifestyle, diet and supplementation:

March 25, 2026 — ApoB: 101 mg/dL

The numbers were moving in the right direction.

Then I continued those interventions and added Zetia on the advice of my physician.

My next measurement:

July 20, 2026 — ApoB: 79 mg/dL

From beginning to end:

110 → 101 → 79 mg/dL

That’s a reduction of:

31 mg/dL

or

28.2%.

And my LDL-C fell by 68 mg/dL, or 44.2%, over the same overall period.

One caveat worth mentioning is that these tests weren’t all performed by the same laboratory. Some of my bloodwork was done through Labcorp and some through Quest Diagnostics.

Ideally, when you’re tracking a biomarker longitudinally, you would use the same laboratory and methodology every time. Different laboratories can use different assays, instrumentation and reference ranges, so some degree of inter-laboratory variation is possible.

But I don’t think that changes the important point here.

We’re not looking at a tiny movement of a few percentage points that could plausibly be dismissed as laboratory variation.

We’re looking at a progression from 110 to 101 to 79 mg/dL, and an overall reduction of 28.2%.

So while I wouldn’t pretend that every individual measurement is perfectly interchangeable between laboratories, the magnitude and direction of the change make the overall trend very clear.

My July ApoB result was also below the <90 mg/dL reference threshold used in my Quest report.

For me, that’s another lesson from tracking biomarkers over time:

Don’t obsess over the precision of a single data point. Look at the trajectory.

But even the clearest ApoB trajectory only tells me about the particles, not how my arteries are actually holding up against them.

So between lab tests, I track that side too, using the CONNEQT Pulse at home to follow measures like central blood pressure, arterial stiffness, and SEVR over time.

So what actually caused the 28.2% reduction?

Now I can give a more nuanced answer than I could before.

I know that lifestyle was helping, because both my ApoB and LDL-C were already falling before I started Zetia.

That’s an important part of the story.

Then I added Zetia while continuing those interventions, and the improvement accelerated substantially.

But I still can’t tell you precisely how much of the total reduction should be attributed to eliminating red meat, reducing saturated fat, increasing fiber, changing protein sources, berberine, better fueling, normal biological variation or ezetimibe.

Nor should I pretend that I can.

This wasn’t a randomized controlled trial.

It was an N-of-1 intervention with a very specific objective.

I wasn’t trying to determine which individual intervention lowered ApoB the most.

I was trying to lower my ApoB and LDL-C.

And that’s what happened.

That’s why I don’t think the takeaway should be:

“Here’s Craig’s ApoB protocol. Copy it.”

The takeaway is the process.

And to be clear, this is my own experiment, not medical advice. If your ApoB is high, the move isn’t to copy my stack. It’s to work it through with your own clinician.

The bigger lesson: it doesn’t have to be lifestyle versus medication

This might be the most useful thing I took from the whole process.

Health conversations tend to turn binary: lifestyle or medication.

You can see it with GLP-1s, where an effective drug becomes a reason for some people to care less about diet, exercise and sleep, as if the medication makes the fundamentals optional.

But that is the wrong framing.

The real opportunity is often lifestyle plus medication. Build the strongest possible foundation through behavior first, then use medication intelligently where it can add incremental benefit.

That was certainly my experience. Lifestyle was already moving my numbers substantially in the right direction. Adding medication didn’t replace that work. It amplified it.

That’s how I think about longevity now.

Not “natural” at all costs. Not medication as a way to avoid changing anything. Not perfection, and definitely not the latest hack. Just objective data, the fundamentals first, honest measurement of whether they’re working, and the willingness, with the right medical guidance, to add another tool when it makes sense.

Seven months on, my ApoB is down 28.2% and my LDL-C down 44.2%. I’m proud of those numbers, but the process behind them is the part that matters.

Measure. Intervene. Measure again. Refine.

And perhaps most importantly: don’t let how fit you look, or how healthy you feel, convince you there’s nothing left to learn from your data.

What to read next

If your own bloodwork has ever surprised you the way mine did, here’s where to go next.

My Cardiovascular Risk Is High, Even Though I’m Fit.
