Available requests
Subject to applicable law, you may request:
- access to or confirmation of personal information or consumer health data;
- correction of inaccurate information;
- deletion;
- a portable copy where applicable;
- a list of third parties and affiliates that received your consumer health data where applicable;
- withdrawal of consent for future consumer health data collection or sharing;
- an opt-out of targeted advertising, sale, or sharing where applicable;
- review or appeal of a denied request; or
- action through an authorized agent.
Website analytics and advertising choices can be changed at any time in the Privacy choices section of the Cookie and Tracking Notice. A recognized Global Privacy Control signal is processed as required by applicable law.
Submit a request
Complete the privacy-request form on this page and select the request type that applies. Submit only the information reasonably necessary for us to identify and verify your request. Do not submit readings, reports, prescription answers, or other medical details unless the form specifically requests them. CONNEQT will provide a secure method if additional sensitive information is reasonably necessary to identify the records at issue or fulfill the request.
If the form is unavailable or inaccessible, email hello@conneqthealth.com with the subject Privacy Request or Privacy Appeal. Do not submit medical details through ordinary email.
You do not need to create a new account to submit a request. We may request additional information when reasonably necessary to authenticate the requester, locate the relevant records, protect another person's information, or comply with law.
We respond within the period required by applicable law. If we deny a request in whole or in part, we will provide the explanation and appeal method required by applicable law.
We do not return health records through a public email thread. Approved records are delivered through a secure method.
