CONNEQT Washington Consumer Health Data Privacy Policy

This Policy explains how CONNEQT, Inc. ("CONNEQT," "we," "us," or "our") handles consumer health data covered by Washington's My Health My Data Act. Consumer health data is personal information that identifies or is reasonably capable of being linked to a consumer and identifies the consumer's past, present, or future physical or mental health status, including information inferred from other information.
1. Categories of consumer health data collected and how they are used
Depending on the product, feature, or website interaction, CONNEQT may collect and use:

2. Sources of consumer health data
CONNEQT may collect consumer health data from:
- the consumer, including through an app, intake, note, questionnaire, support request, or privacy choice;
- a compatible CONNEQT product or a CONNEQT app feature used by the consumer;
- a connected health service expressly authorized by the consumer;
- a healthcare professional or another person when the consumer directs the transfer or that person is legally authorized to act for the consumer; and
- the consumer's browser or device when the consumer interacts with a covered CONNEQT website page, product, cart, or purchase-confirmation function after the required collection choice; and
- an advertising platform when the consumer submits its native form to request a CONNEQT educational program.
3. Categories shared and recipient categories
CONNEQT shares the following categories of consumer health data when necessary to provide a product or service requested by the consumer, with the required separate consent, at the consumer's direction, or as otherwise permitted by law:

Specific affiliates with which CONNEQT shares consumer health data: CONNEQT Health Limited (formerly Cardiex Limited); AtCor Medical Pty Limited; AtCor Medical Inc; and Cardiex (Shanghai) Medical Technology Co., Ltd.
4. Website collection and sharing choices
For a consumer reasonably identified as being in Washington, CONNEQT presents two separate, unchecked choices before the covered activity begins:
- Site collection permits collection of limited page, product, cart, campaign, purchase, device-family, coarse-geography, performance, error, form, and navigation activity that may indicate health interests to measure performance. This includes cookie-free aggregate reporting and optional detailed analytics.
- Partner measurement separately permits disclosure of approved limited health-interest website or conversion signals to advertising, affiliate, or campaign-measurement providers for measurement, optimization, or targeted advertising.
The second choice is separate from the collection choice. Rejecting or withdrawing either choice stops the corresponding future covered collection or sharing. A privacy notice, acceptance of Terms, silence, continued use, or consent to collect is not treated as consent to share.
CONNEQT uses available state information supplied by the consumer, associated with an account or transaction, or derived at a coarse state level from the network connection to determine when to present the Washington choices. We do not request precise location for this purpose. If a reliable Washington signal becomes available later, future covered collection and sharing remain blocked until the applicable Washington choices are obtained.
A recognized Global Privacy Control signal does not create consent and suppresses covered external advertising or targeted-advertising disclosures. CONNEQT does not load an advertising provider or send an add-to-cart, purchase, or other advertising event for a consumer treated as being in Washington unless the separate collection and sharing choices both permit it.
The cookie-free aggregate system does not create a visitor, session, customer, order, or cross-page history. It excludes raw IP address, raw user-agent string, visitor or session identifier, order or cart identifier, advertising click identifier, form value, search term, contact information, account information, prescription information, and support content. CONNEQT does not treat raw or reasonably linkable activity as de-identified merely because it is later combined into a report. When CONNEQT treats information as de-identified, it takes reasonable measures designed to prevent association with a person or device, publicly commits not to attempt re-identification, and contractually requires a recipient to comply with those restrictions.
5. Consumer rights and how to exercise them
Subject to the Act, a consumer may request to:
- confirm whether CONNEQT collects, shares, or sells consumer health data concerning the consumer;
- access consumer health data concerning the consumer;
- receive a list of third parties and affiliates with which CONNEQT shared or sold the consumer's health data and available contact information;
- withdraw consent and stop future collection or sharing, subject to legal exceptions; and
- delete consumer health data concerning the consumer.
Submit a request or appeal through the Privacy Request page or email hello@conneqthealth.com with the subject Washington Consumer Health Data Request or Washington Consumer Health Data Appeal. CONNEQT may take reasonable steps to authenticate the request. A consumer may change the website Site collection or Partner measurement choice in the Privacy choices section of the Cookie and Tracking Notice.
