CONNEQT VIP Signup Offer Terms

CONNEQT, Inc. ("CONNEQT") offers eligible consumers a 10% discount in exchange for joining its VIP marketing-email list (the "Offer"). These terms state the Offer's material conditions.
1. Current Offer
Provide your first name, last name, and email address through the approved CONNEQT signup form to receive a code for 10% off an eligible public direct-to-consumer CONNEQT PULSE purchase, including an eligible public package or bundle that contains CONNEQT PULSE. For an eligible package or bundle, the discount is calculated on the entire eligible merchandise amount of that package or bundle, not only the PULSE device component.
The Offer does not apply to a private or Blueprint offer, a purchase that does not include CONNEQT PULSE, a standalone accessory, standalone Care+, an assessment pack, shipping, tax, or another fee. There is no minimum purchase or maximum dollar discount. The code may be used once per customer and email address and cannot be combined with another coupon, discount code, or private or special offer.
A code expires on the date stated when it is issued. If no expiration date is stated, the code remains valid until CONNEQT withdraws it, subject to applicable law. CONNEQT may end or change the Offer prospectively and may withdraw an unused undated code, but will not reduce a discount already applied to a completed eligible order except for fraud, error, cancellation, refund, chargeback, or as required by law.
A return or refund is based on the amount actually paid and remains subject to the applicable Return Policy, including any permitted fee or adjustment. A code is reissued only if the order is canceled before fulfillment or the code or order failed because of CONNEQT's error; it is not reissued after an ordinary return.
2. Email enrollment and optional SMS
By selecting Claim Your 10% Off, you request marketing email and accept these Offer Terms and the Consumer Dispute Resolution Agreement for disputes connected with this Offer and enrollment, including its individual binding-arbitration and class- and jury-waiver provisions and, subject to its terms, the one-time 30-day arbitration opt-out right following first acceptance.
A telephone number is not required for this Offer. Marketing SMS enrollment is a separate optional choice governed by the SMS Program Terms. Declining SMS does not affect eligibility for the email Offer.
3. Personal information involved and use
The Offer involves identifiers and contact information—first name, last name, and email address—together with the enrollment, code, redemption, email-interaction, unsubscribe, and suppression records reasonably needed to administer and evaluate it.
CONNEQT uses this information to issue and administer the discount, send requested marketing email, prevent fraud or duplicate use, maintain preferences and suppression, and evaluate the Offer. Communications and commerce providers may process the information for CONNEQT under applicable restrictions. CONNEQT does not authorize use of the submitted contact information for targeted advertising.
4. Opting in and withdrawing
You opt in by voluntarily completing the approved signup form and selecting Claim Your 10% Off after having the opportunity to review these terms.
You may withdraw at any time by using the unsubscribe link in a marketing email or contacting hello@conneqthealth.com. Withdrawal stops future marketing email after processing and does not require returning a properly redeemed discount. CONNEQT may retain a limited suppression record so it can honor the withdrawal.
5. Privacy and choices
The Privacy Notice explains CONNEQT's practices and available privacy rights. Participation is voluntary. CONNEQT will not unlawfully discriminate against a consumer for exercising a privacy right, but a person who does not provide the information required for the Offer will not receive the associated signup discount.
