CONNEQT SMS Program Terms

These SMS Program Terms ("SMS Terms") govern optional recurring marketing text messages sent by CONNEQT, Inc. ("CONNEQT") through the CONNEQT Health marketing program (the "Program").
1. Eligibility and enrollment
The Program is offered only to a person who is at least 18 years old and provides a U.S. mobile number that the person is authorized to use. Enrollment is optional.
By checking the Program's SMS box, providing your mobile number, and submitting the form, you electronically sign and authorize CONNEQT to send recurring advertising or telemarketing text messages to that number using an automatic telephone dialing system or other automated technology. Consent is not a condition of purchasing any property, goods, or services.
Entering a telephone number without checking the SMS box is not marketing-text consent. The VIP email offer and discount remain available without SMS enrollment, subject to their own terms.
2. Messages and charges
Messages may include CONNEQT offers, promotions, product news, and general educational content. Message frequency varies. Message and data rates may apply under your carrier plan. Carriers are not responsible for delayed or undelivered messages, and delivery is not guaranteed.
Marketing messages are not medical advice, emergency alerts, or a substitute for professional care. CONNEQT does not use the Program to send device readings, app health records, report details, assessment results, prescription information, or other individualized health content.
3. Opting out and help
You may revoke consent at any time. Reply STOP, QUIT, END, REVOKE, OPT OUT, CANCEL, or UNSUBSCRIBE to an incoming message, or clearly communicate the request through another reasonable method, including by contacting hello@conneqthealth.com or +1 234-266-6378.
CONNEQT will honor a valid revocation within the period required by law and no later than ten business days after receipt. It may send one nonmarketing confirmation of the request. After opting out, reply HELP or contact the methods above for assistance; a new affirmative enrollment is required before marketing texts resume.
4. Your number
You represent that you are the subscriber or customary user of the number and are authorized to enroll it. If you stop using or control of the number changes, opt out before the change or promptly notify CONNEQT. CONNEQT may suspend messaging when it reasonably believes a number is invalid, reassigned, ineligible, or associated with misuse.
5. Privacy
CONNEQT handles the submitted number, consent record, message activity, and suppression record as described in the Privacy Notice. Communications providers may process this information for CONNEQT under applicable restrictions. CONNEQT does not authorize use of the submitted telephone number or message content for targeted advertising.
6. Disputes
The Consumer Dispute Resolution Agreement affirmatively accepted with Program enrollment governs disputes connected with the Program, including its individual binding-arbitration and class- and jury-waiver provisions and, subject to its terms, the one-time 30-day arbitration opt-out right following first acceptance.
7. Changes or termination
CONNEQT may change or discontinue the Program prospectively. A material change to these SMS Terms or the scope of marketing consent will receive the notice or renewed consent required by law. Ending the Program does not remove a suppression record reasonably retained to honor an opt-out.
